What to Expect

First time at the clinic?

Welcome to Dynamic Chiropractic and Sports Therapy Center! 

Each new patient will start with a health intake and a detailed assessment. Assessments may involve observing movement and posture, taking the patient through a complete movement screen and measuring range of motion, motor/sensory/neural testing, and utilizing specific orthopedic tests.

  • Upon scheduling an appointment you will receive a welcome email to our online booking patient portal (when you click this you get to the portal) as well as a link to fill out a new patient intake form. This form is required for all new patients and new injuries (and for patients that haven’t been to the clinic in 2 years).
  • If you have already filled out your intake form online please arrive 5 minutes prior to the start of your first appointment for our administrative staff to confirm your information, and to enjoy a cup of Stone City Coffee on us!
  • If you are unable to fill out our online intake form please arrive 15 minute prior to your first appointment.
  • Our entire team is here specifically for you! Enjoy feeling better!
